Required Forms for Participation
To ensure a safe, organized, and successful athletic experience, student-athletes at Traders Point Christian Schools are required to complete specific forms prior to participation. These forms help our staff support students, communicate clearly with families, and meet league and school requirements.
Forms may be required before the start of each season and may vary by sport and level. Families are encouraged to review and submit all required documents in a timely manner to avoid delays in participation.
Access and Submit Forms
Athletics forms include items such as physical examinations, consent forms, and participation agreements. Submission methods and deadlines may vary and will be communicated prior to each season.
Please review all form instructions carefully and ensure that documents are completed and submitted by the required deadlines. If you have questions about a specific form, athletics staff contact information will be provided within the form materials.
All forms are completed online through Eventlink when registering for a sport. Physicals will need to be completed prior to registering. You may download a physical form to take with you to the physician for your appointment. Once that form is filled out and signed by the physician, you will submit it to Eventlink during registration.
